Partial Hospitalization
PARTIAL HOSPITALIZATION PROGRAM
Our Partial Program is designed to teach you skills that help you better manage the symptoms of your illness. We provide supportive therapeutic environment in which you can prepare for your return to your community, job or school.
Treatment Team
While attending the Partial Hospitalization Program (PHP), you will be treated by a team of mental health clinicians which include a psychiatrist, registered nurse, licensed clinical social worker, adjunctive therapists and chemical dependency counselors. These clinicians will provide group and or individual interventions to help you meet your treatment goals.
Treatment Plan
When you first arrive in the program, the nurse will ask you about your symptoms and what you would like to accomplish in PHP. This information will be recorded in your medical record and will be used to formulate a problem list and goal list, which we call a treatment plan. The nurse will review the list with you and ask you to sign this form.
Attendance
PHP operates Monday - Friday 9 a.m. to 2 p.m. Van service is provided for those who need it and who live within a 20-mile radius of the hospital. If for any reason you are unable to attend the program as scheduled, you need to contact the RN at (626) 859-5294. If we are transporting you, you will also need to contact the van driver to tell him/her not to pick you up. The phone number to call is (626) 966-1632 ext. 265 and let Kandy Adams know. It is important to communicate the reason for your absence to the RN. (Four unexplained consecutive absences might result in administrative discharge from the program).
Participation
While you are attending PHP, you will be expected to attend and verbally participate in your groups. Any written assignments given in groups or for you to take home should be completed. These assignments are designed to teach cognitive behavioral techniques, to help you work on reducing your symptoms. Your psychiatrist may prescribe medications to further effect your symptoms. You should take your medications as discussing this with our doctor first. If you are seeing any other doctors, be sure that they are aware of medications that you are taking, likewise, if they prescribe any medications you should inform your psychiatrist. Periodically during your treatment the RN may ask you for a urine sample for drug screening, in addition, blood work may be ordered to check that your medications are within therapeutic levels. Your family can plan an important role in your recovery. It is important to include them in your treatment when possible. Family therapy and family groups are available to assist in this process. It is our policy to contact only those members of the family that your have indicated should be contacted. (You may revoke your permission to contact family at any time during your stay).
Meals
Lunch will be provided daily, free of charge in the cafeteria. The cafeteria is unavailable to PHP at all other times. The PHP staff will provide coffee. Regular coffee should be limited to 1 cup per person; decaf coffee and ice water are also available. We ask you to limit your caffeine intake (coffee, cola, chocolate, and tea) as caffeine has been found to aggravate symptoms like anxiety and sleep disturbance.
Lockers
Lockers are available to store your personal items. You must provide your own lock.
Refrigeration
If you choose to pack your own lunch or snacks and need to store them in the refrigerator, please make sure all items are clearly marked with your name and the date. Items will be thrown away after 5 days. No items should be placed in the freezer.
Treatment Team
We believe a team approach is the most effective method to deliver quality
care. The treatment team is led by a Board certified MD/Addictionologist and
is made up of Certified Alcohol and Drug counselors. Each patient's needs
are assessed prior to starting treatment in order to assure proper placement
and address all recovery concerns. The treatment team offers not only their
clinical expertise and experience, but also their concern and compassion.
Referral
Private and confidential assessments and referral information are provided at no charge 24 hours a day, seven days a week through Aurora Charter Oak Hospital's Intake Department.
